Application Forms
We understand that entering a nursing home can be a stressful time for the person involved and their family. We promise to do everything we can to make this process as simple and worry free as possible. Our admissions procedure is very straightforward. The application forms can be downloaded from this web site or are available from the Hospital office.
There are two parts to the application form the first is a medical form that can be filled out by the applicants GP or Hospital Doctor. The second is part is a social form and can be completed by the applicant, a relative or friend.
The Director of Nursing, the Hospital Manager and Hospital GP will review the forms. Admission is based on our ability to provide the appropriate level of care for the applicant. The applicant will be invited to the Hospital to meet us and have a look around. A tour of the Hospital can be arranged by contacting the office. New residents are welcome to initially stay for a trial period.
Weekly fees are available on request.
